CAMPER MEDICAL FORMS

Campers who are bringing medications with them to camp must have them noted on their registration.  If your child is bringing an inhaler or epi-pen, please complete and turn in the apppropriate form below to Summer Program Office Staff no later than the Wednesday before your child's first day of camp.  These forms MUST be stamped by your child's physician in order for them to attend camp. This in accordance with State of Maryland summer camp regulations.  Campers will not be permitted to camp if this is not followed.

 

If your son or daughter requires medicine other than an Epi-Pen/Inhaler, parents must administer these before camp.  If necessary, parents may administer medication to their child in the main office throughout the camp day.
